The following forms and information are for TOWN EMPLOYEE USE ONLY:

If you have moved/changed your mailing address, changed your email address, or changed your phone number, please use the form within your NeoGov forms section of your Dashboard.

If you have recently changed your name and need to change it within the town systems, please use the form within your NeoGov forms section of your Dashboard.

If you need to change information regarding your direct deposit, please use the Form Process through your NeoGov account. It will ask you to attach a voided check or a Direct Deposit Authorization Form from your bank.

The following link is for Police/Firefighters who need to report an injury on the job:

 

 If you are planning on resigning/retiring from the Town of West Springfield, please fill out the Town Separation Notification Form on your NeoGov Dashboard under Forms.

If you are taking a leave of absence (any absence longer than 4 days), please fill use the FMLA Leave Request Process on your NeoGov Dashboard under Forms.
